Not because of old age, but I've seen them fail when nothing else in the PSU shows any signs of failure. Corsair has too. That's why even cheaper units like the CX have a Japanese primary now.

Remember, the PSU fan speed is controlled by a fan controller. Another fan may be just as loud.

The AXi does OCP just like any other multiple +12V rail PSU. The only difference is that its controlled/monitored by an MCU as opposed to a supervisor IC.
